Description
Installed FreeNAS 11.3U4.1 and 11.3U5 on two servers with the same hardware configurations:
Manufacturer: Supermicro
Product Name: SSG-6028R-E1CR12H
Memory: 32 GB RAM
Storage: 12 x 8TB SAS drives (including OS drive).
After rebooting the servers 3 - 5 times we get the following boot error:

ZFS: i/o error - all block copies unavailable
ZFS: can't read MOS of pool freenas-boot
gptzfsboot: failed to mount default pool freenas-boot
FreeBSD/x86 boot
ZFS: i/o error - all block copies unavailable
