complete system lockup

This has been going ever since I upgraded to TrueNAS core 12. Every 7-10 days I get all kinds of errors thrown from middlewared then everything becomes unavailable and I have to perform a hard reboot. This last time I caught the problem right in the middle of it happening with I/O errors on my pool call Main causing the system to eventually lock up and force a hard reboot. I have tried installing truenas fresh on a new boot volume and setting up from scratch. Problem still returns. I have run memtest several times on this machine, so I don't suspect the hardware. Any help is greatly appreciated. Thank you so much.
