FreeNAS fails to create alert for failed power supply that IPMI does detect as failed

Description

I have a FREENAS-CERTIFIED-2U-12 purchased from iXsystems. It's running FreeNAS 11.3-U5 (current newest).

I recently updated the IPMI firmware, and noticed by coincidence while in the IPMI web interface that it was reporting one of the power supplies as failed.

(I created support Ticket ID: FVP-445-61031 and happily received a new PSU under warranty.)

This bug is to say however that FreeNAS does not generate an alert for this failure, neither in the FreeNAS UI itself nor by email. I generated a debug output and in IPMI/dump.txt the failure is noted, ex:

PS1 Status | C8h | ok | 10.88 | Presence detected
PS2 Status | C9h | ok | 10.87 | Presence detected, Failure detected

PS1 Status | 0x1 | discrete | 0x0100| na | na | na | na | na | na
PS2 Status | 0x3 | discrete | 0x0300| na | na | na | na | na | na

But the detection at this low level does not make it up to the UI somehow.

I am returning the defective unit to you, hopefully you can reproduce with it.

I still have the defective unit installed BTW, so if there are some commands/tests you'd like me to run, I believe I have 30 days before I need to return the unit to you.  Or we could do the opposite and I could return it ASAP so you can try to repro with it.  I guess it depends how likely it is to repro in another server...

Unfortunately, the full debug report contains too much private information (I see our employee and customer names in there for example), but I can provide a subset. Attached are the Hardware and IPMI folders. Let me know if other parts would be helpful.

Thank you for the report .  Can you please provide a debug by navigating to System -> Advanced, click save debug, and upload it to the Private attachments area in this ticket?
